If you are travelling abroad please make sure you contact us in plenty of time to arrange any vaccinations that may be necessary. To help the Travel Nurses assess your travel needs it is important that they are in receipt of the assessment form before your appointment.

Please complete this form 8 weeks before your travel date. You must obtain travel vaccine information for the country/ies you will be visiting from the Travel Health Pro website (www.travelhealthpro.org.uk). Travel Health Pro provides a consolidated travel health report for multiple destinations and is continuously updated by their team of qualified health professionals.

Please contact the surgery to collect your form 7 days after your submission and to check the advice the practice has given regarding new vaccinations or boosters required.
